Why is an open wound look deeper now then it was after surgery?

It may be: Open wounds can get larger initially. Normally after the skin is opened there is fat under it. Blood supply to fat is poor. It may dry out and die, so the wound base ends up being muscle instead of fat.

Optical Illusion: When you have a surgery and an open wound is left, there is considerable swelling (edema) of the tissues which may make them look closer together. As the inflammation resolves, the swelling does so also and the depth may appear to increase. This is because the tissues have less water and so appear smaller and deeper.
