Person to person: Contact with infectious material from an person with smallpox is how this was passed. There are no insects or animals that spread the germ. Since there has been no cases since 1977, we only hear about this from people worried it may resurface as a bioterror weapon.

Smallpox: is spread by respiratory droplet, like by a sneeze or cough. The virus, however was destroyed by the vaccine introduced in the 1940's, and was eradicated from the earth in 1980. Aside from a sample kept in a lab for research purposes, smallpox as a disease in the wild no longer exists. Hope this helps!
