Alopecia totalis: Intralesional or topical corticosteroids are the initial treatment for most patients with patchy alopecia areata. Topical immunotherapy can be used as first-line treatment for patients with extensive disease in which greater than 50% of scalp hair is lost. Some second line therapies include minoxidil, anthralin, and photochemotherapy. Systemic therapy is usually reserved for refractory cases.
