See your doctor: There are many things doctors consider when recommending "best" treatment. Generally it comes down to a blood thinner (there are several) vs a filter (also many types) to prevent more big clots from going to the lung. The doctor must carefully assess the risks and benefits on an individual basis. "best" treatment is a case by case assessment.

Inferior Vena Cava Filters are generally reserved for patients that have failed anticoagulation (blood thinners) or some reason they can't take them (bleeding ulcers, etc).
