Simple stuff first: There are very few downsides to trying prolotherapy. You can try prolotherapy and if it does not work then consider other options for arthritis such as knee replacement. Knee replacement is the last option and should be considered after several other arthritis treatment options have been exhausted. There is no going back to prolotherapy after tkr.
