See a surgeon: Simply finding a vein(cephalic or basilic) in your arm and forearm with good size , isolate it , disconnect it and suture it to a close by artery do it becomes bigger and thicker . But u really need to talk to your surgeon in person to explain more.

Depends: Sometimes it's just a single small incision with vein brought over to the artery. Other times two or more incisions of great length are needed. Discuss your case with your surgeon.
