A member asked:

Will the doctor do tests after my baby has a uti?

Yes: Typically in a young infant who has the first UTI associated with fever, tests are done make sure there is no reason why it may happen again. A renal ultrasound is done to check the kidneys & bladder to make sure there is no anatomic deformity. A VCUG study is a test that involves inserting a tiny catheter into the bladder to make sure there is no reflux of urine that goes back towards the kidney.

Yes: After a baby has a uti, the doctor may do urine tests to be sure the infection is gone. He may also do an ultrasound scan to see if there are birth defects in the kidneys and to see if the kidneys are the proper size. Another test is a voiding cystourethrogram, which is an x-ray study that looks at the bladder, to see if the urine goes backwards from the bladder up towards the kidneys.

Yes: Most pediatricians will do a test of cure which is a second culture. Most of us will also perform a special xray called a VCUG and many will perform an ultrasound of the kidneys. Uti's are a major source of kidney failure for babies so you've got to be careful. Watch http://www.Youtube.Com/watch?V=tggfht6jrm4 and http://www.Youtube.Com/watch?V=1nbwkiug8vi.

Yes: Ultrasonogram of kidneys is done to detect whether the kidney(s) is obviously enlarged or infection started inside the kidney is spreading out to form an abscess (perinephric), usually done once the diagnosis is established. Voiding cystourethrogram, often done after 1-2 weeks of antibiotic treatment, detects urine backflow ( from bladder toward kidney) while infant/child is voiding urine.

Yes: It does depend upon the age and sex of the child. Under 2 years tests will generally e done to assess if there is a birth defect or damage from the ifection which predisposes to further infection. These tsts are done in any boy, preteen with uti. Older girls are tested if they have more than 1 infection over a short period of time.

Yes: Good question. When an infant has a uti, we get concerned that there might be something anatomically that puts them at risk. Pediatricians will often order imaging tests to evaluate this further- typically a kidney ultrasound and vcug. The ultrasound is looking for obstruction of urinary flow and enlarged kidneys. The VCUG looks for reflux of urine from the bladder back to the kidneys.

Yes: If your child is less than 12 months old, your provider should perform both a renal ultrasound and a voiding cystourethrogram (VCUG) in order to determine that the urinary tract is without malformation. The recommendation to get a VCUG is being debated currently, but it is still considered to be the best practice. Older kids will get tests depending on gender (boys are always tested) and severity.

Yes: Yes if your child is a boy. Boys don't get urinary tract infections easily. Girls have a much shorter bladder outlet (urethra). Therefore, its easier for a girl to get a bladder or kidney infection without an underlying problem in her kidneys or bladder. However, each case should be treated individually.

Yes: Besides repeating the urine, good likelihood they will send you for a kidney ultrasound and a test called a VCUG test to check for reflux of urine going from bladder to kidneys.

Yes: There is a lot of controversy these days on testing after uti. Most physicians order an ultrasound of the kidneys. If this is normal, it's the first uti, and baby is over one month of age, some doctors may hold off on the vcug, in which a catheter is inserted and contrast injected to look for backwards flow of the urine to the kidneys. A culture of the urine should always be done.

Yes: Depending on the age of your child your doctor may do a renal ultrasound to identify any anatomical problems that could have predisposed to the uti. Your doctor may also obtain a vcug, a test that checks for the presence of urinary reflux, the back-flow of urine from the bladder to the kidneys, which can also predispose to urinary tract infections. Other tests may follow depending on those results.

Hopefully: Baby should have a follow-up urinalysis plus kidney & bladder ultrasound. I also like to get a voiding cystourethrogram xray, after 3 -4 weeks, if baby had a febrile UTI to rule out reflux. New academy of pediatrics guidlines for evaluating utis in children recommended against a work-up after single uti, however most pedi. Urologists disagree & would advise above studies before a recurrence.

Yes: A UTI in a baby, especially with fever, needs a work up including kidney and bladder ultrasound.
