Many: The most obvious sign of a hyperosmolar hyperglycemic state is an alteration in mental status or functioning. People may become lethargic or drowsy, confused, or even delirious. In severe cases a person may become unresponsive. Occasionally they can have seizures, visual problems, and movement or sensory problems. This is a dangerous condition that need immediate medical attention.

Confusion: Hhnk is common in the elderly diabetic, or can be the 1st presentation of diabetes in the older adult. Often somnolence and confusion are the only complaint, but can be accompnaied by a fever if there is an infection(but not always). It is a medical emergency requiring aggressive hydration and close monitoring in a hospital setting.
