Bleed - menses: it is fairly common to have spotting before expected menses. If you have severe pain associated with menses and dark blood prior to menses I would suspect endometriosis.

Yes & no: Yes if: it's just your period. Women's bodies are allowed to tweak themselves once in a while. No if: was right after sex, or came from your urethral or anal regions. If you are taking birth control pills, variability is more unusual.
