Puberty: I presume you have seen an obstetrician regularly through this problem. Were there any "bugs" that needed antibiotics ever found? Anxiety disorder is "painful". Have you seen a Psychologist. If not, please set up an appointment. That will ease at least some of your troubles and hopefully all of them.
