Brain cancer: In general, Stage 4 means distant spread to other organs other than the organ where the tumor started (lung cancer that spreads to brain, would be Stage 4 Lung Cancer). There are 4 stages, so Stage 4 is the most extensive degree of spread, unfortunately. Primary brain tumors are not staged, because they rarely spread. Prognosis is based on age, location, histology, functional status, extent, etc
