Many things...: This could be associated to many different things not necessarily herpes especially on the arm. It is not usual to find a herpes related blister/cyst as a solitary lesion. Usually this type of virus presents in clusters. If we are talking about shingles could be on the arm but once again in clusters, painful, burning and can be itchy. See your doctor to have this lesion seen, diagnosed and treated
