Penicillin: Penicillin (by injection or intravenously) is the first choice treatment. In fact, syphilis is just about the only medical condition for which penicillin still is recommended. For people with serious allergy to penicillin, a tetracycline antibitotic (usually doxycycline) is used, or ceftriaxone if the brain or spinal cord are involved (neurosyphilis).
