Doctor suspects a thinning pad. walking without applying pressure to the heel (heel not touching ground surface) also hurts. is this really pad issue?

Plantar fasciitis??: It is very rare to have a heel pain due to a thinning of the fat pad. The majority of patients have pain due to plantar fasciitis and or a heelspur in this area. If you are putting all your weight on the ball of your foot and the heel still hurts, then that could be a tight plantar fascial ligament too.
