What's been tested?: If you were my patient I would get blood work - CBC, CMP, lyme test, ESR. I would have you see a cardiologist first. If cardiac is normal, then a pulmonary specialist. If lungs are normal, then an ENT. There is a cause for your symptoms so don't give up on trying to find it. Eat sensibly - small high protein meals every 3-4 hours.

Hard to say.: May be from varying causes.
I would suggest to see a integrative holistic doctor.
