No one knows.: IPF is primarily a fibrotic disease of the lung whose cause is unknown. Most people do not survive 5 years beyond diagnosis. Since we do not know what causes IPF it is hard to know why it progresses faster in some. Poor prognostic factors are older age, big tobacco history, low body weight, and extensive disease on imaging.
