Uncommon at all: These will occur when the skull bone is fractured and the membrane covering the brain is torn. Surgery can do it , spinal puncture or epidural anesthesia. Trauma through the ear can do it as well. Some surgery through the nose can do it as well. If you know someone who has had head trauma make sure they see a doctor.
