For chronic swelling: it may be helpful unless infection has become effiectively treated and infection has not spread from the epididymis into the testicle. The inital therapy should be prolonged antibiotic therapy. Surgery would be a last resort without any guarantee of a pain free life after surgery. I suggest you confer with a urologist.
