Several things: The first is to have a sigmoid or colonoscopy. Magnesium 200-400 mg before bedtime would be good to try as it relaxes muscles. I have had success with a digital exam working on relaxing the muscles in the area. The other is called Neural Therapy or nerve blocks to the sacral plexus.

Ouch!: Since ano-rectal spasm, while very painful, doesn't last long, medication isn't too helpful. Sitting on "the throne" and straining as if you were trying to have a bowel movement may help. It works the same way as flexing your ankle does if you get a Charlie Horse in your calf: by actively stretching the contracted, cramping muscles. What you describe is typical for levator ani syndrome.
