Infection: I would recommend a urinalysis and and STD screen for gonorrhea and chlamydia if you have been sexually active. If not it could be a UTI or prostatitis.

Most likely cause of penis pain? Started as mild discomfort when peeing but now urinating is fine, the base of behind the penis hurts, even worse when erect. Feels like a persistent 'fractured bone' kinda feeling. No discharge etc?
