Maybe: coughing increases pressure in the abdomen. Increasing pressure in the abdomen pushes down on the internal inguinal ring. This is where the spermatic cord leading to the testicles enters the abdomen. if inguinal hernia is present coughing can make the hernia bulge more and cause discomfort felt in the scrotum. Get examined by a physician
