What happens if i had a time machine, what vaccinations would i need to go back to medieval times?

Same as now: You would need the same ones we give today along with the smallpox vaccine if you could find one. Your understanding of germs and the value of clean drinking water and sanitation would help you avoid illlness.

Hard to say: The genetic makeup of all organisms changes over time. Modern vaccinations are based on modern micro-organisms. They might not work against medieval strains. Also, there are some common microbes to which every generation develops immunity, but to which travelers (in space or time) would find lethal. So just having modern vaccinations & hoping for the best could be a fatal miscalculation.
