Hard to say: The genetic makeup of all organisms changes over time. Modern vaccinations are based on modern micro-organisms. They might not work against medieval strains. Also, there are some common microbes to which every generation develops immunity, but to which travelers (in space or time) would find lethal. So just having modern vaccinations & hoping for the best could be a fatal miscalculation.
