HIV transmission: Although use of a condom greatly reduces the risk of HIV transmission, it is still possible. There can be breaks in the condom or bodily fluids can make contact in areas that the condom does not cover. A man can be infected with HIV from a blow job, although the rate of infection is less than with unprotected vaginal intercourse.

Possibly : Yes to both. But it is an exceedingly rare occurrence. Open sores in the mouth would be required and condoms are quoted to bring something along the lines of 99% percent effective. But remember if you fall into that 1% or even .1% it's a 100% for you. Be safe.
