It might be: Have you recently delivered? Breastfeeding? Do you have a fever? Nipple discharge? This is potentially a sign of mastitis (soft tissue infection of the breast) or even breast abscess (soft tissue infection with collection of pus). See your provider as soon as you can. If you have a fever you may even want to go to the ER.
