Not too often: considering that there is a clear statistical association between even second-hand smoke and the development of cancer, bronchitis and cardiovascular disease, I don't think there is any data on what amount of smoking could be "safe." I have had a very few patients who would have a cigarette on special occasions, perhaps one every several months, who did not have any detectable adverse effects.
