See details: Are you seeing a rheumatologist? Has a blood test been done to check if the HLA-B27 antigen is positive? There are several extremely effective therapies for this issue. With a history of diarrhea the sacroiliitis may be related to inflammatory bowel disease. I strongly urge a rheumatologic consultation if one has not already been done.
