Tubal Ligation: You might consider getting your tubes tied. There are no hormones and you don't have to remember something every day. Another option would be a Paragard 'Copper-T' IUD. If you can quit smoking, other options would be available.

Tubal ligation: My first question would be how many pregnancies have you had and the number of living children. If you feel you that you want no further pregnancies and you have risk factors against using several other forms of birth control, as you do, I would consider a tubal ligation. It is possible to do this under general anesthesia and via the laparoscope. You will have a much shorter recovery time.
