Orchiectomy first...: Some transgender women have orchiectomy (removal of the testicles) because they cannot tolerate the effects of testosterone blockers like spironolactone. Testicles usually shrink anyway under the influence of estrogen; after their removal the scrotum "flattens." Talk to your surgeon, and be sure to be conscientious with your vaginal dilation after surgery. LGromkoMD
