A pathogen is a germ: So whatever pathogen you pick - you can read about 'what' diseases they cause. Influenza virus for example, causes 'the flu' which is influenza infection. One germ can cause more than one disease too - for example 'varicella' or the chicken pox germ can cause chicken pox, but it can also cause 'shingles' or herpes zoster. You might want to look on wikipedia for 'general' information of this sort.
