Location issue: If you are talking about right behind ear, there usually aren't major lymph node chains hanging out unless it is more in the neck region. Something that may feel like lymph node in the area may be blocked duct/follicle. Not sure if this has anything to do with high prolactin levels but this would not be typical. You can try warm compress as if this is a blocked follicle, may help.

May represent Ca: Large nodules in the upper posterior triangle of the neck may represent malignant lymph nodes as a primary tumor or metastatic disease from the posterior oropharynx. A head dand neck, oral exam is needed followed by a biopsy, prolactin levels probably unrelated .Since in males it is associated with a benign prolactinoma of the pituitary.
