It is a type of arthritis which causes joint swelling and destruction due to deposition of formed crystals that come about from an enzyme defect in the body. Typically, however, the joints that are most frequently affected are those in the feet and ankles with other joints being affected later on.

Many: Although one of the first toe joints is the most common, other toes, the mid foot, ankles, knees fingers, wrists, and elbows can be involved. Shoulders and hips are extremely uncommonly involved with gout. There are rare reports of back joints effected by gout.

lower extremity: Most of lower extremity joints are affected in gout, very frequently affects the big toe, but can affect the entire joints of the feet, ankle and knees. Also can affect other joints, including upper extremities: hands, wrists and elbows. In general the first attack of gout is in one joint only, termed called monoarthritis.
