Different: Fissures and fistulas are different things. A fissure is a tear in the skin of your anus while an anal fistula is a "tunnel" that communicates the inside of the anus with the "outside" anal skin. They both can produce anal pain, but pain is more typical of an anal fissure. Anal fistulas can give you recurrent anal abscesses, chronic anal discharge, etc ... I hope this helps.
