Could be anything: But most likely its an infection due to fungus. You can try over the counter anti fungal local application and improve hygiene. If it does not get better in few days , please see your pcp.

Rash: Sounds like you have a rash in your groin and anus. Get eval. Could be something like a simple fungal infection. Best wishes.
