Could be anything: But most likely its an infection due to fungus. You can try over the counter anti fungal local application and improve hygiene. If it does not get better in few days , please see your pcp.

Rash: Sounds like you have a rash in your groin and anus. Get eval. Could be something like a simple fungal infection. Best wishes.

STD: This can indicate that you have a sexually transmitted disease. One such possibility is molluscum contagium, this can be treated but you should see your local urologist or health clinic for an evaluation.
