Cortisone?: I'm assuming you're getting cortisone injections. If you're getting relief for your condition from the injection, then one every six months is reasonable. You should be able to continue to get them, as long as they're 6 months apart.

Lube or steroid?: If "synvisc, " can forestall surgery for quite a while, if still getting good time between shots (=months). If dwindles to wks/days, tells you is time for surgical eval. If steroid, usually by third in same joint, consider surgery. Talk to your othopedist for advice in your specific situation.

If bone on bone: May do more damage to the bone and it makes it harder to do a knee replacement. Depending on your age and medical problems see if a knee replacement is an option, discuss it with an orthopedic surgeon.
