That's right: 2 weeks are added so the gestational age is measured form a theoretical last menstrual period, never from conception.

Yes: Dating of pregnancy is confusing to many patients but when you are told you are 6 weeks pregnant the baby was not created 6 weeks ago but 4 weeks ago. So technically when you are 6 weeks pregnant you are 4 weeks from the day the egg was fertilized with sperm.
