Probable not: A couple of benign conditions can cause the type of lesion you are describing. However, it may continue to bleed if traumatized. Best to have it removed and examined by a pathologist to be sure.

Usually no: Need more history, and details: trauma, how big, previous sore or discoloration, etc. If it truly just appeared, it sounds like a "blood blister" or something similar, which is usually benign and temporary. However, some serious skin problems are associated with bleeding, so if it continues to be a problem, you should have it checked out.

Not likely: Sounds like you have a blood filled blister and this is most probably the result of trauma. Your body has formed a natural band-aid and if you can, leave it alone for a few days. It will rupture on its own eventually. Try to prevent further trauma to the site; specifically try to prevent a lot of friction so that the skin can heal painlessly.
