Some risk: There is a small risk of getting pregnant after getting your tubes tied, but it's not zero. If it happens, it's most likely within the first year after you get them tied and it's a very high risk that, rather than a normal pregnancy, it will be an ectopic or tubal pregnancy. If you've had your tubes tied and you're worried about being pregnant, talk to your dr.

Yes: Fallopian tube ligations/cautery/clipping does have a small faliure rate based upon the method used to alter the tube. Also, many tubal ligation failures are ectopic pregnancies as the sperm can get to the egg, but the fertilized ovum can't get back through a partially obstructed tube to implant in the uterus and will "stick" wherever it gets "stuck".
