Why would an obgyn suggest a laparotomy over a laparoscopy for a 15cmx8.4cmx15.7 pelvic cyst ? i am 20 years old. i'm concerned about my fertility .

That is a big tumor: A cyst of this size has a fairly high risk of being cancerous. Because of that the doctor wants to be able to look around inside all of your belly to make sure that there is no spread of cancer. Ask him to explain why he is proposing to do what he has told you. He could certainly do laparoscopy to start with and convert it into a full laparotomy if he finds any indications that the cyst is cancer.
