There is no one best: Treatments vary from patient to patient. Most of my patients do not need biologics and can be successfully treated with older drugs, but combinations are needed. Sometimes we give rescue doses of certain tnf-alpha inhibitors to provide comfort until other drugs "kick in." but humira, enbrel, cimzia, rituxan, orencia, (abatacept) xeljanz are always costly alternatives! mtx should be tried first!

Biologicals: The treatment of rheumatoid arthritis has really improved over the last decade since introduction of new class of medications called biological they have many side effects and need to be selected properly for best fit for individual patient please talk to your rheumatologist who will be the best one to answer your specific question good luck.
