Anal fissue: Theoretically, yes. If a vaginal birth is complicated by a 4 th degree tear, then that potentially can weaken the sphincter and a concomitant sphincterotomy could aggravate that. Sphincterotomy alone and in the right hands has a minimal chance of sphincter issues and is an excellent treatment of chronic anal fissure that has failed medical mgmt.
