Easy: Consult with a dermatologist. I think from experience that patients who try to rid them selves of "blackheads" with over the counter products or trying to squeeze them out get into trouble and make things worse.

Acne: Acne is a disorder of the sweat glands. Excessive sebum production, and defective 'keratization' leads to obstruction of normal flow of sebum on to the skin surface. Comedo is formed. Open comedo is also called "black head " black heads are although unsightly, are easily managed. The contents of the open comedones easily escape to the skin surface. Consult your doctor for treatment.

Exfoliate: Black heads are sebum producing skin cells exposed to the air to help black heads, you can try otc products that exfoliate such as gentle scrubs, Alpha hydroxy products. If this does not work, you can go for a medical grade microderm or peel. They work well and really help with skin care product penetration.
