Vascular or nerves: Diabetes can cause the nerves to get damaged causing a "neuropathy." this can be felt as a tingling, numbness, cold sensation. Issues with your blood vessels (vascular disease) is also common in diabetes, which can cause feeling of hot and cold, pain at times, as well as numbness, etc. You need to follow-up with your dr. To get evaluated for PVD and or neuroapthy. Best wishes.
