Pain, tenderness.: Usually, epididymitis is painful while testicular cancer presents classically with a painless mass. Fever, tenderness and swelling can also be present. The major cause in heterosexual men < 35 years old is chlamidya trachomatis, accounting for 70% of cases. A course of appropriate antibiotics clears it up. Testicular cancer can be worked up by taking AFB and b-hcg levels, chest x-ray, and ct.
