Needs evaluation: I would recommend that you see your doctor for any new or changing lesion on your skin or scalp , especially if it is bleeding randomly and you don't recall any injury. Good luck.

Infection or cyst: Pilar cyst could be there for years without causing any symptom. Folliculitis (hair follicle infection) with or without abscess could be another. See a dermatologist.
