See doctor: He may just have an irritation of his nipples if he is a runner.However without seeing the rash one cannot be sure and he should see a dermatologist or general physician.

See doctor: If it is itchy yet appears infected, it is time to see a doctor to find out. Hydrocortisone cream may help the itching but an antibiotic may be needed for the infection . Even more important-finding out what might have caused the rash in the first place.
