Don't need to: These are normal findings for men and do not require treatment. If they are bothersome, the only way to get rid of them is by surgical excision. Topical creams creams are not indicated for this.

Ablative treatment: Pearly penile papules usually require an ablative therapy to destroy the lesions. Cryotherapy, electrodessication, surgical excision, and carbon dioxide laser have all be used. Carbon dioxide works the best and has the best cosmetic outcome.
