Depends: I'm assuming that this question relates to pericardial effusion, the condition of having fluid next to or around the heart. When we think of fluid "under the heart" we are usually referring to (in colloquial terms) fluid inferior to the heart. This region should in general easily be visualized by an echocardiogram, but in certain cases it could be difficult to see.
