Venous stasis: And I am guessing he has a fair amount of swelling as well. Sounds to be advanced venous stasis dermatitis although diabetes can give you a superimposed dermopathy. Seems young to have this. Hopefully he has had evaluation for cardiac sources of edema.

How swollen?: He should see his dr now. Sounds like vascular or arterial or lymphatic disease or all 3. Treatment is a function of exact diagnosis. Does he spend long hours sitting or standing? Other meds? Pain or itch or fever? Prior leg problems? Using any meds on legs?
