When it doesnt work: Once you can eat any amount of food and have stopped losing or are gaining weight, you may be a candidate for a revision. Talk with your surgeon about a revision. Having done the first bypass, he will know better than anyone what to use as parameters for a revision.

Big pouch, outflow: The pouch does not usually stretch out. Above picture is the outflow from the pouch. When it is this big, some narrowing will help.
