It depends: Chronic external hemorrhoid tags generally do not cause any pain at all, unless a blood clot has formed inside, in which case the pain can sometimes be severe, but usually resolves after 1-2 weeks. Internal hemorrhoids frequently cause mild to moderate discomfort which can often persist until treated. There are other causes of anal pain as well.

Hemorrhoids: External hemorrhoids can hurt but should not bleed. Internal hemorrhiods should not hurt, but can bleed. Some patients don't have hemorrhoids at all, but instead have a fissure or tear of the rectal lining that hurts.
